Appointments and Office Hours

Our office is open Monday through Friday, 8 a.m. to 5 p.m. For emergency care after office hours, please call our office number (402) 397-7989 or (712) 325-0014. These numbers are answered 24 hours a day. A physician is available for emergencies 24 hours a day.

Billing/Insurance Information

At Adult Pediatric Urology & Urogynecology, P.C., we accept most major insurances, Medicare and Medicaid. We encourage all patients to confirm their insurance coverage prior to treatment. Please bring your insurance card and referral, if necessary, to your appointment. We also encourage you to arrive early to your first appointment to complete all necessary paperwork. Please bring a list of all current medications and any pertinent lab and x-ray results or medical documents you have.

As a courtesy to our patients,we will submit your insurance claims for you. It is important that you provide us with the name, address and telephone number of your insurance company and policy number. Since insurance coverage is a contract between you and your insurance company, it is your responsibility to remit payment for your balances not covered by your insurance company. Our fees reflect the usual and customary range for our region and indicate the level of care which is provided to you. If your insurance plan requires you make a co-payment or pay co-insurance, please do so at the time of your visit or you may be asked to reschedule your appointment.We accept cash or check. If you are unable to make a payment, arrangements can be made by contacting one of our billing representatives at 402.399.7888.

Medical record information will not be released without prior written consent. Some reports or copies of medical records require payment in advance. Please allow two weeks for this service.

CareCredit

It is important to be able to schedule treatment and care right away. That's why we offer CareCredit, a healthcare credit card specifically designed to pay for treatments and procedures not covered by insurance. CareCredit has two unique features. Every CareCredit transaction is eligible for a No Interest (if paid within promotional period) or Low Fixed Interest Payment Plan – and, you can use the card at all healthcare practices that offer CareCredit. Hospital Affiliations

Adult Pediatric Urology & Urogynecology, P.C., is a private practice group. We have privileges to admit patients and provide care at most local hospitals for your convenience.
